Each student is responsible for the maintenance of this check-out sheet.  Please try to balance the number of check-outs with any one faculty member over the course of the semester.  This sheet must be turned in to the course instructor prior to the final examination.  Failure to perform a check-out or turn in this sheet by May 2, 2002 will result in a grade reduction.

CARDIOPULMONARY EXAMINATION AND TREATMENT
PT 534 Clinical Applied Physiology
Final Practical - Spring 2001

Student:

Patient Problem Number:

Scoring is based on a 5 point scale. Specific point values and the performance criteria are as follows:

Score

Criteria

0

Assessment or treatment item not performed and was essential given the patient problem, and/or list of signs and symptoms.

1

Strongly Disagree

The student did not demonstrate minimal competency in the performance of the skill item and required 2 or more verbal cues or frequent examiner redirection to complete the skill item.

2

Disagree

The student did not demonstrate minimal competency in performance of the skill item and required 1 verbal cue or verbal redirection to successfully complete the skill item.

3

Neutral

The student demonstrated minimal competency in performance of the skill item.

4

Agree

The student demonstrated competency in the performance of the skill item.

5

Strongly Agree

The student met and exceeded the expected level of competency in the performance of the skill item.

A score of 4 in each of the categories is equal to a grade of 100 for the section. The grades from each of the sections will be averaged to achieve a final score for the practical. The practical consists of 3 sections and the grade on the practical counts toward 20% of the final grade. To successful demonstrated content mastery, the student must meet the minimum score in ALL sections. Not achieving the minimal score for any section will result in a re-examination on all sections. The final grade will be determined by averaging the grades from all examination attempts at achieving content mastery.
